All users of the facilities of Acton Community Access Television, Inc., are required to sign the membership agreement below each year. Theprocedure for filling out the form is as follows:
- Read the agreement on the form below. The full Policies and Procedures are available as a PDF document.
- Click in the box labelled "Click to sign" and type your name. A legal electronic signature will be generated for you, or you may draw your signature with your mouse, track ball, trackpad, or other input device.
- Click "Apply"
- Move from field to field with TAB or by clicking on the next field.
- Enter your street address.
- Enter your town, state, and zipcode.
- Enter your telephone number.
- On the same line as the telephone number, enter your own email address.
- Only if you are under 18 also enter the email address of a parent or guardian who will sign for you. (See below *.)
- Click on the "Click to eSign" button.
- A confirmation link will automatically be sent to your own email address.
